Foot Extensor Tendonitis (Tendonitis at the top of the foot)


Extensor tendinitis is an inflammation of the extensor tendons - these are the long, thick cords that attach to your toes and pull them up toward your shin.

What Are Tendons?

Tendons are vital structures connecting muscles to bones. In the top of the foot extending to the toes, there are a set of tendons, called extensor tendons, which are responsible for the dexterous movements of the toes. They are made up of tissue called collagen and elastin, ensuring that the tendon is both tough but flexible.

Tendons need to glide smoothly, allowing the powerful contraction of the muscle to move the intended bones and joints, and hence are enclosed by synovium, a thin sheath of smooth tissue. When the tendons become irritated, their ability to glide within these compartments is restricted, causing top of foot pain during movement and tenderness when direct pressure is applied.

Tendon fibers can tear apart in the much the same way a rope becomes frayed. Once a tendon becomes frayed, an inflammatory response is triggered; over time an inflamed tendon can become thick, bumpy and irregular making it more difficult to glide during toe movements. Without proper rest and time to heal, a damaged tendon can become permanently weakened.

Symptoms of Foot Extensor Tendonitis

If you are suffering from tendon damage in the extensor tendons of the foot, you will feel pain and tightness in the top of the foot. You will also likely feel pain when you stand up and walk or run, especially when walking uphill or up stairs. If the tendonitis is substantial, you may experience significant swelling and possibly discoloration in the top of the foot. If you flex the foot or try to raise your toes upward, the pain will increase in intensity.

There are other tendons and muscles extending from the toe areas (more for finer control) that could be strained as well, in which case you may also feel:

  • pain in the ball of the foot
  • pain in the arch of the foot
  • pain in the outer edge of the heel or even under the heel

If much of the pain experienced in beneath the arch and/or at the bottom of the foot, this could stem from flexor muscle or tendon strain (used to curl the toes down) or perhaps a plantar fascia injury.

Anyone can suffer from tendonitis, but it's most common in adults due to degeneration of tissue as we age. Over time the tendons in the foot will wear down resulting in something called degeneration. This is where the fibers in your tendons will become more weak - it's just a natural process that happens as we age. Any abnormalities in your feet like higher or lower arches (flat foot) can result in foot tendonitis as well.

Causes of Foot Extensor Tendonitis

Overuse

This is the most common cause of foot tendonitis. Tendonitis in the foot due to overuse is most often linked to either poor footwear or repetitive activities often found in sports.

Tightness at the heel or midfoot may lead to tension on tendons that pass through these areas, causing pain and inflammation.

Trauma / Foot Injury

An injury such as a foot fracture or sprained ankle can, in some cases, tear extensor tendons.

Tight Achilles / Tight Calf Muscle

Tightness in the back of the lower leg can cause extra tension on extensor tendons in the foot. Over time, the tension will eventually cause strains and tearing in the tendons.

Calcium deposits in and around the foot or ankle joint is another cause of foot tendonitis, and is often referred to as Calcific Tendonitis.

Extensor tendons connect muscles found in 1) the lower leg and 2) the back of the foot to the top of the toe bones. The muscles pull on the tendons which cause the foot and toes to extend upward. The tendons run through a series of rings, or pulleys, that form tunnels along the toes. They work similar to a line guided along a fishing rod, with the pulleys holding the tendon close to the bone. These tendons are made up of tissue called collagen and elastin, ensuring that they are both tough and flexible.

Extensor tendons need to glide smoothly and are enclosed by synovium - a thin sheath of smooth tissue that reduces friction for easier movement. When the tendons become irritated and inflamed, their ability to glide within these compartments is restricted. This causes pain and tenderness when direct pressure is applied. When the extensor tendons are inflamed it is referred to as extensor tendonitis (also sometimes spelled tendinitis).

An injury to these tendons can make tasks you do on your feet difficult, and may very well affect a persons performance or abilities at work. At present the results of tendon repair surgery are far from ideal, as such an invasive technique can often create excess scar tissue (adhesions) between the sheath and the tendon, restricting mobility and leading to a loss of function. Basically, the tendon NEEDS to glide smoothly within the sheath; if there is scar tissue (adhesions) gluing the sheath and tendon together, these adhesions must be broken for the tendon to work smoothly again. This is why physical therapy is often underestimated; a physical therapist is trained to deal with problems affecting range of motion issues and scar tissue is often a big component of their focus.

Prevention of foot extensor tendinitis is exercised simply by not overdoing any activities that use a upward foot motion. (ie. driving, hiking, walking/running up hills, certain types of sport) Be careful about suddenly increasing the time spent on these activities, as your tendons are likely not properly conditioned to the added stress. Consider paying close attention to the positioning of the foot in certain situations - try to maintain the foot in a neutral position and avoid any excessive flexing.

Foot Tendonitis Conservative Treatment Options

If you have tendinitis in your foot, rest is highly recommended. Try to stay off your feet, avoid activities that cause pain or may have caused the inflammation and begin cold compression treatments to reduce swelling as soon as possible.

During the healing process, scar tissue builds on the damaged tendons; this scar tissue can cause the tendon to thicken, decreasing the area in tight spaces (such as the top of the ankle), increasing the chance of secondary conditions such as ligament strain or nerve impingement.

In addition, know that scar tissue attaches to everything. It can bind tendons, ligaments, muscles, bone and even skin together wherever it grows. Inflexible scar tissue will limit movement of your soft tissue very quickly. Tendons and muscles that were once elastic are now constrained - if you stretch it too far it will start to rip, causing spikes of pain. As a result, movement of the entire joint is limited, making it difficult to flex your foot or even walk. At the very least, scar tissue leaves a tendon weaker than a healthy tendon, leaving the injury at greater risk of further deterioration and possible tearing.

The trick to any tendon injury is getting it to heal with minimal scar tissue formation and with as much realignment of tendon fibers as possible. To achieve this, your doctor will most likely recommend up to 6 months of conservative treatments. Even with optimum healing there is always less elasticity in a previously injured tendon. The trick is to make sure you heal it the best you can, that way your chance of re-injury down the road is much lower than average - which is over 50%.

All connective tissue is living tissue; tissue requires a blood supply and is continually in a state of regeneration. Any and all connective tissue has the potential for injury and inflammation. Injuries can be small such as in the case of a strain, or injuries can be large tears or even avulsions (complete tearing). Once an injury is experienced, the area will experience both inflammation and the growth of scar tissue (otherwise known as adhesions).

The occurrence of scar tissue is a regular part of life. Scar tissue forms as part of the progression after injury occurs. Scar tissue essentially 'fills in the gaps' after a cut, tear or wound presents itself in the body. Scar tissue is composed of the protein, collagen; it is widely known in the medical community that scar tissue is weaker and less flexible than original tissue.

  • Normal/Original connective tissue is made up of collagen, but this collagen is aligned in a specific direction with each strand in alignment. This results in the connective tissue having a strong tensile strength while also having excellent flexibility and extensibility.
  • Scar tissue is made up of a 'criss cross/ random' formation of collagen and attaches to everything in the area. You can likely imagine the result of a patch of scar tissue; the effect is similar to pouring extremely sticky glue in your hair. It interrupts everything and is disruptive to the function of the connective tissue. The caveat is that the scar tissue does its job - it binds tears together. You definitely want the tear to heal, but it is important to ensure the scar tissue goes away despite the fact that it does not go away easily.
  • After scar tissue forms, although the wound or cut is now closed (and less prone to infection), the area of tissue itself is inferior in functionality to what it replaced.

Continuous re-injury and build-up of scar tissue makes it more likely that you will wind up with chronic pain, reduced Range of Motion or even arthritis (permanent damage).

The human body will use scar tissue as a temporary solution and will try to build the scar tissue as fast as possible to heal tears in soft tissue (muscles, tendons). Scar tissue can form fast to bring together the edges of a tear, but working fast doesn't mean that the job's done right. When scar tissue forms it doesn't come together as neatly as regular (healthy) tendon/ligament tissue would.
