Plantar plate injuries are often misdiagnosed as hammertoes, neuromas, or capsulitis (inflammation of the tendons that surround the toe joints). A misdiagnosis means the true underlying condition isn’t treated and the pain and deformity will continue to worsen.

One of the most common presenting problems, yet most commonly missed problems, is a plantar plate tear. Often the problem is considered to be potential neuroma, fat pad atrophy, or a generalized diagnosis of metatarsalgia or metatarsal head overload. Unfortunately, not enough attention is placed on the plantar and medial/lateral ligamentous structures of the metatarsal-phalangeal joints. This lack of attention results in poor diagnosis, lack of care, treatment for the wrong condition, and ultimate frustration for the patients and doctor.

If you are suffering from pain in the ball of your foot, it is important to ensure you get a correct diagnosis quickly. There are two reasons for this:

Plantar Plate Function & Location

Your foot is a complex area of the body, it's small compared to the rest of your body but it's packed full of tendons, ligaments, muscles and bones. At the base of each toe in your foot there is a small fibrous band of tissue that connects foot bones to toe bones. The plantar plate(s) are a special type of ligament that is super strong and flexible (because you need something like this to handle your bodyweight). The job of these plates is to help pull your toes down toward to the floor. The plate helps stabilize your toe from shifting side-to-side and maintains the toe in its correct position. Each foot contains four of these plantar plates - one for each toe except the big toe, which does not have a plantar ligament plate.

Causes of Plantar Plate Tears

Plantar plate injuries may happen suddenly through an acute incident / accident / trauma or develop over a period of time from over-use or from wearing shoes with little sole support. They can also result from a deformity that can put increased pressure under the foot (ie. bunion or crossed toe).

A Specific Trauma Incident

An example of such an incident could be tripping over a root or perhaps a cat that crossed your path. When you tear your plantar plate this way, you will feel sharp stabbing pain across the ball of the foot.

Chronic, Repetitive Stress

Rather than the plantar plate tear occurring all at once, the tear can develop over quite a length of time due to repetitive substantial stress on their toes and the balls of their feet. A common cause of chronic plantar plate tears is from the added stress on the plate caused by an over-pronated gait (when you walk, your feet roll inward); over time, the plate begins to bread down.

As stresses mount, the plantar plate suffers from micro-tears in the tissue and gets stretched out, becoming more and more vulnerable to larger tears. Age is also a very significant factor; the plantar plate wears down with age. Furthermore, connective tissue weakens and becomes less flexible with age. If you are an active adult over the age of 45, you can get a plantar plate tear although people that spend a lot of time on the balls of their feet are much more susceptible. A few examples of professions that have significant susceptibility to plantar plate tears are:

  • People who spend a lot of time on ladders: painters, linesmen, tree fruit harvesters
  • Ballet dancers. Ballet dancers dedicate hours and hours per day strengthening and toughening their bodies - especially their feet; a bad foot injury can easily end their career.

How Do I Diagnose a Plantar Plate Tear?

It's important to find out what's wrong with your foot. Plantar plate injuries are often misdiagnosised as they are similar to some other injuries that affect the foot. This is very true for those who are very active or enjoy running sports. When seeing your doctor they will discuss your medical history and examine your foot, heel and ankle for any deformities or inflammation (swelling, redness, heat, pain). They will also look at your flexibility, stability, and gait (the way you walk). Occasionally an x-ray of your foot will help rule out Metatarsal (toe) Stress Fractures. An MRI can be used to provide a complete and accurate diagnosis of any injury to the soft tissue (ligaments) in your toes. In some cases the doctor will request blood tests to rule out diseases or infections.

Plantar Plate Tear Symptoms

Pain & Lack of Cushioning Sensation

As mentioned above - if you are suffering with plantar plate tears you may feel as if you are "walking on the bones of your foot". You will likely have pain at the base of your 2nd, 3rd or 4th toe (usually the 2nd) that worsens with walking, jumping or running. Pain varies between patients with some describing a dull ache type of pain while others experience sharp pains.[1] A tearing and or popping sensation is sometimes felt when the injury occurs due to the plate tearing or the toe dislocating and 'popping' back into place.

There is a lot of pain involved with a plantar plate tear so if you have one then you are probably limping to lessen the pain. Many sufferers of this injury notice swelling and redness on the top of their foot and if the injury is severe there could be bruising near the affected toe.

If left untreated, you will develop a toe deformity called 'hammer toe'.

Position Shift of the Affected Toe

Once a plantar plate tear occurs, the affected toe (usually the 2nd) will begin to gradually shift position. The toe may shift upwards or move toward one side; it is not uncommon to see a plantar plate tear affect the 2nd toe where it shifts position toward the big toe. The affected toe will often not touch the ground when the foot is planted.

Do You Really have a Plantar Plate Tear?

Our feet are some of the most complicated joints in our bodies with 26 bones, 33 joints and more than 100 tendons, muscles and ligaments. Due to this, there are often a variety of ailments in the foot that can cause similar symptoms. Some ailments can be extremely serious, so if you are experiencing persistent foot pain, please visit your physician and get a diagnosis.

Toe pain can come from injuries to the soft tissue in the foot or to one of the many tiny small bones, some of the more common injuries being:

  • Foot bursitis - Bursitis can occur in the toe joints, the side of the foot, the heel and around the ankle. As we walk, run or jump, the ground surface and the shoes we wear play an important role in how much trauma our feet experience. Every step can cause a small amount of damage to a particular area in the foot which can increase the risk of bursitis.
  • Morton's neuroma - This condition usually causes burning pain in the ball of the foot that may radiate into the toes. The pain generally intensifies with activity or wearing shoes. You may sometimes feel that you are "walking on a marble," and you have persistent pain in the ball of your foot. You may also have numbness in the toes. Neuroma is often the misdiagnosis of plantar plate tears the two conditions have similar symptoms.
  • Metatarsalgia - This condition can also cause pain in the ball of your foot, that area between your arch and the toes. Metatarsalgia is a common over-use injury, persistent stress can lead to chronic irritation and inflammation of the bone covering and surrounding tissues, such as ligaments and tendons.
  • Sesamoiditis - The big toe has two very tiny bones embedded in tendon tissue in the ball of the foot. Any type of injury to the tendon tissue can cause irritation in and around the sesamoid bones. This condition can also cause pain in the ball of your foot, that big toe. Sesamoiditis is a common over-use injury, persistent stress can lead to chronic irritation.
  • Plantar Fasciitis - Pain associated with plantar fasciitis normally starts as a dull ache and then progresses to a sharp, knife-like pain or constant throbbing feeling that is worse when you put weight on your foot. The most tender areas tend to be in the center of your heel and along the inside and bottom of your foot towards your toes, where your heel and arch meet.
  • Turf toe - This can happen when the big toe joint is pushed beyond its' normal range (backwards or downwards). A 'pop' is felt at the moment the injury happens followed by sharp immediate pain. Symptoms include swelling in the foot, the entire big toe joint is affected and limits the motion in the toe.
Predislocation syndrome, crossover toe deformity and floating toe syndrome are terms related to a plantar plate tear or sprain.

Plantar Plate Tear Treatments

The first thing your physician will tell you is to stop doing whatever it was that caused this problem in the first place. This is a big problem for many people that work on their feet, because it usually means you'll have to take time off from work to heal. For cases of significant severity, it is advised is to stop all activity related to the injury and rely on someone else to help you. If this sounds like you then you know how important it is to heal your plate tear quickly and make sure it doesn't happen again.

Regardless of the degree of severity of your plantar plate tear, it is a good idea to start the following conservative treatment options as soon as you can:

  • rest the foot as much as possible
  • avoid activities that cause pain
  • getting fit for custom orthotics or shoe inserts with metatarsal pads is recommended
  • wear shoes with solid support along the ball of foot area
  • if there is inflammation, use cold compression treatments to reduce swelling
  • visit a podiatrist (foot doctor) or physician to get a proper diagnosis and evaluation

If a plantar plate tear is not cared for properly, there is a good chance that such an injury will become chronic. Once in the chronic stage, you will experience a toe deformity and likely suffer further degradation of soft tissue and cartilage in the joint. Little can be done to fix a chronic plantar plate tear other than surgery.

For Low Grade Plate Tears (minimal injury)

A change in activity, rest and better shoes & orthotics might be all that is needed to recover from your plantar plate tear. It is also possible that your physician will prescribe a boot to wear that is designed to keep your weight off of the ball of the foot. However, if there is a significant change in toe alignment (vertical or horizontal) then it is likely that more will be needed.

For Higher Grade Plate Tears

A higher grade tear means that there is substantial tearing of the plantar plate - enough to affect toe alignment. While it is good to undergo the conservative treatments outlined earlier, it will likely not be enough. Once your doctor or foot specialist has evaluated the severity of your tear, they may recommend a range of treatment options such as:

  • Toe taping. Tape may be wrapped around the toe to keep it in a specific position - often in a downward position. Whatever way it is wrapped - it will be done to keep pressure off of the plate tear, giving it time to heal. Toe taping can last for a couple of weeks to a couple of months.
  • Surgery. The surgical procedure undertaken will depend on a couple factors such as:
    • is there degradation of cartilage and ligaments in the joint?
    • Is the plantar plate too damaged to repair? In such cases, a graft procedure might be recommended.
    • How much collateral damage (damage to surrounding tissue and bone) is there? Although open surgery is more invasive than arthroscopic surgery, open surgery might be necessary to perform a proper cleanup of the area. This means recovery will be longer and the amount of scar tissue due to surgery will likely be more significant.

Scar Tissue & Fibrotic Adhesions

All connective tissue is living tissue; tissue requires a blood supply and is continually in a state of regeneration. Any and all connective tissue has the potential for injury and inflammation. Injuries can be small such as in the case of a strain, or injuries can be large tears or even avulsions (complete tearing). Once an injury is experienced, the area will experience both inflammation and the growth of scar tissue (otherwise known as adhesions).

The occurrence of scar tissue is a regular part of life. Scar tissue forms as part of the progression after injury occurs. Scar tissue essentially 'fills in the gaps' after a cut, tear or wound presents itself in the body. Scar tissue is composed of the protein, collagen; it is widely known in the medical community that scar tissue is weaker and less flexible than original tissue.

  • Normal/original connective tissue is made up of collagen, but this collagen is aligned in a specific direction with each strand in alignment. This results in the connective tissue having a strong tensile strength while also having excellent flexibility and extensibility.
  • Scar tissue is made up of a 'criss cross/ random' formation of collagen and attaches to everything in the area. You can likely imagine the result of a patch of scar tissue; the effect is similar to pouring extremely sticky glue in your hair. It interrupts everything and is disruptive to the function of the connective tissue. The caveat is that the scar tissue does its job - it binds tears together. You definitely want the tear to heal, but it is important to ensure the scar tissue goes away despite the fact that it does not go away easily.
  • After scar tissue forms, although the wound or cut is now closed (and less prone to infection), the area of tissue itself is inferior in functionality to what it replaced.

Continuous re-injury and build-up of scar tissue makes it more likely that you will wind up with chronic pain, reduced Range of Motion or even arthritis (permanent damage).

The human body will use scar tissue as a temporary solution and will try to build the scar tissue as fast as possible to heal tears in soft tissue (muscles, tendons). Scar tissue can form fast to bring together the edges of a tear, but working fast doesn't mean that the job's done right. When scar tissue forms it doesn't come together as neatly as regular (healthy) tendon/ligament tissue would. Scar tissue fibers will lay down over top of your tear in a cluttered, messy and jumbled up way.

This is how scar tissue works - it is messy and inelastic. The scar tissue that forms in your soft tissue will be unorganized and won't line up properly with the healthy tissue surrounding the tear. This scar tissue will attach to everything in and around the tear including the surrounding healthy tissue as well. This results in a long-term fusing together of everything the scar tissue touches, reducing your ability to move and making the injury even more painful!


For soft tissue injuries (tendons, muscles, ligaments, fascia), it is important to heal quickly and completely; this will help minimize the build up of scar tissue. If you don't, your tissue injury may be the start of a long term chronic injury resulting in a loss of Range of Motion and joint atrophy.

The longer a soft tissue injury remains, the more scar tissue is likely to build up in and around the injury location. This is why it is critical to treat your injury quickly without delay. Continuous re-injury and build-up of scar tissue while staying active or going back to work means you will experience a greater risk of winding up with on-going, chronic foot pain and overcompensation injuries to the hips, knee(s), and or back.
