Fascia: What Is It?


Much like tendons and ligaments, fascia is tissue that is made up of several extremely thin layers of collagen fibers that are all oriented in the same way. In between each layer of fascia, there is a liquid (called hyaluronan) between each layer. This liquid helps fascia stretch without friction as you move. Unfortunately, inflammation can dry up the liquid hyaluronan layer, causing friction and damage when the fascia should move.

In some places, fascia is very thick, while in other places, fascia is very very thin (almost transparent). Due to the way these fibers are oriented, fascia is designed to be very flexible and capable of withstanding very large forces. Do not underestimate the importance of fascia; it is attached to basically all connective tissue, running from the top of your head to end of the foot.

Fascia within the body is absolutely critical; arguably the most obvious function of fascia is that it helps hold our skeletal, muscular and nerve structure together.

Fascia surrounds individual muscles, muscle bundles within individual muscles, groups of muscles, blood vessels, and nerves. It binds these structures together in much the same manner that PLASTIC WRAP is used to hold a huge deli sub together.... Among the different kinds of tissues that are involved in the body’s "elastic" framework, fascia has received the least scientific attention — probably because in most regions of the body it cannot be diagnostically imaged with even the most technologically advanced imaging techniques such as MRI ... Nevertheless, fascia plays a major role in joint stability / instability, proprioception, coordination, strength, joint motion, flexibility, as well as CHRONIC PAIN SYNDROMES of all kinds.

"Proprioception, otherwise known as kinesthesia, is your body’s ability to sense movement, action, and location. It’s present in every muscle movement you have. Without proprioception, you wouldn’t be able to move without thinking about your next step. Proprioception allows you to walk without consciously thinking about where to place your foot next. It lets you touch your elbow with your eyes closed."[3]

Fascia is located throughout the inside of your body. It attaches to and stabilizes every muscle, tendon, ligament, bone, organ and tissue in your body.... fascia is a part of a system-wide network that provides form and function to every part of your body. It’s a continuous layer of tissue that’s flexible and able to resist tension.

Types of Fascia

There are 4 major types of fascia:

  1. Superficial Fascia

    This is the outermost fascia layer, and is found directly beneath the skin. Superficial fascia is thicker in the core of the body while thinner in the legs and arms.

  2. Deep Fascia

    This fascia layer is thicker than superficial fascia and covers muscles, bones, nerves, cartilage, tendons and flood vessels. Deep fascia has two components:

    • Aponeurotic fascia (thick and pearly white in color)
    • Epimysial fascia (thinner than aponeurotic fascia, separates from muscles less easily than aponeurotic fascia)
  3. Visceral Fascia

    This fascia surrounds your heart, lungs and organs in your abdomen.

  4. Parietal Fascia

    This fascia lines the walls of body cavities, such as the pelvis.

What Does Fascia Do?

There are many functions of fascia; to start with, fascia provides structure and support throughout the whole body. It bundles and holds muscles together, much like a girdle. Even within a muscle, it maintains the proper position of nerves, individual fibers and blood vessels so they maintain proper placement during movement. It provides a smooth surface for muscles, joints and organs to slide against each other so there is little friction and no tearing. Fascia helps prevent injury by evenly distributing forces across muscle structures. Fascia also has the responsibility of helping to stabilize joints and improve circulation.[1][2]

When Does Fascia Become A Problem?

Fascia becomes a problem when its ability to stretch is compromised. This can be caused by either an excessive thickening of fascia or scar tissue - or a combination of the two. As stated earlier, fascia is normally structured with its cells aligned in parallel fashion; this allows the fascia to be very flexible when demands are placed on it, then return to its static form once forces upon it are minimized. In some ways, you could liken fascia to something like a slinky; when you pull either end of a slinky, it has the capacity to greatly expand, then return to its original shape once you let it go.

Much like a slinky, there is a maximum capacity of expansion for fascia. If fascia is stretched too far, tiny microtears start to form. Once microtears occur, this is the start of trouble. Scar tissue grows due to the tissue tears; unlike your original fascia, scar tissue is inflexible and does not grow in a parallel structure. This means that you will experience loss of flexiblity in the fascia once scar tissue starts to grow; you will not be able to stretch the fascia as far as you could before without introducing MORE microtears and MORE scar tissue as a result.

If we keep with the "slinky" analogy, once you start tearing the fascia, the fascia will not quite return to its original shape. Further to this, the scar tissue will attach in random fashion, much like dropping a very strong glue across the slinky bands. Once you try and stretch the slinky, the glue will keep the bands from expanding and as a result, the slinky will not stretch like it once did. This is similar to the effect that scar tissue has on fasia - you definitely do not want scar tissue in there. There are few ways to get rid of scar tissue, however the easiest option is to ensure you have minimal scar tissue growth to start with. As more microtearing occurs, scar tissue proliferates and your fascia will thicken from scar tissue growth.

Fascia Thickening

It should be apparent to most, that a thickened fascia will be less flexible compared to a non-thickened fascia. If you are experiencing a thickening of your fascia, you will find that you will be experiencing a loss of flexibility. If you start fighting against this loss of flexibility and overstretch, you will experience microtearing in the thickened fascia; this microtearing, as stated earlier, begets scar tissue. Thus you can see the relationship between fascia scar tissue and fascia thickening - they are key components of a vicious cycle that results in continuous loss of flexibility.

The concept of fascia thickening is becoming a very interesting area in the medical field. A lot of time has passed in the medical field where the importance of fascia's relationship to chronic pain has largely been overlooked. Recently, there has been more focus in this field, as some studies have shown links between fascia and some chronic pain syndromes. Moreover, there has been a lot of recent work to determine why fascia thickening is becoming more common now than it has in the past.

Scar Tissue

It has been shown in studies that injury begets inflammation; in nearly all cases of inflammation, there is growth of scar tissue. When comparing scar tissue to "normal" tissue, there are multiple distinctions:

  • scar tissue is weaker
  • scar tissue is less elastic
  • scar tissue will reinjure much more easily
  • scar tissue is more pain sensitive

"...when a nerve link to a part of the body fails, that part of the body becomes highly irritable and extremely excitable. ...the sensitivity in denervated structures increases not just by a small amount but sometimes by over 1,000 times. Subsequently, they demonstrated that supersensitivity can occur in practically every part of the body including skeletal muscle, smooth muscle, spinal neurons, sympathetic ganglia, adrenal glands, sweat glands, and even brain cells. Neuropathic structures over-react to a wide variety of inputs including stretch and pressure. Supersensitive pressure receptors are the reason why neuropathic muscles are tender."[4]

This means that in injured tissues, pain "supersensitivity" can happen, resulting in a possible increase in pain sensitivity by over 1000 times. Injury frequently injures the micro nerve branches that exist in the tissue itself. This is the really the source of hypersensitive issues resulting from injury and scar tissue. Once these nerves are damaged or stretched, there may very well be substantial pain sensitivity in the area where there was very little previous to the injury. When dealing with fascia - the single most pain sensitive tissue in the body - you can start to see why there is a link between overstretched fascia, scar tissue and chronic pain.

According to Dr. Gunn, scar tissue has the propensity to develop abnormally large numbers of immature nerve-buds post-injury — sort of like the hundreds of small twigs you sometimes see sprouting from the cut branches of topped trees. This phenomenon has the potential to make scar tissue over 1,000 times more pain-sensitive than normal tissue. Now add in the fact that FASCIA ITSELF CAN ACT AS A NERVOUS SYSTEM and you can see a train wreck in the making.

With the onset of scar tissue and fascia microtearing, there will be an inevitable restriction in joint range of motion. For example, fascia microtears in the upper back and neck due to whiplash will restrict your ability to turn your neck from side to side or up and down, or both. You will likely experience significant pain during neck movement, possibly also manifesting as headaches as well. As movement becomes more and more restricted, pain continues to manifest, seriously impacting your day to day living with little possibility of going away until the fibrotic adhesions (scar tissue) are broken. Such an example touches on the importance of working on breaking the resticted flexibility and restoring range of motion. The breaking of scar tissue adhesions is one of the critical functions of physical therapy; it is generally agreed, however that the number and frequency of physical therapy sessions are too few to result in a successful outcome for most cases. This should underline the importance of using conservative therapies including prescribed stretches and heat applications every day when you are not at physical therapy.

Heal Quickly to Minimize Scar Tissue Growth & Reduce Re-Injury Risk

If you have microtearing in the fascia it is important to maintain flexibility and heal quickly and completely; this will help minimize the build up of scar tissue. If you don't, your tissue injury may be the start of a long term chronic injury resulting in a loss of Range of Motion and joint atrophy. This is where pro athletes have the advantage. They use therapeutic tools to speed their healing so that they are 100% healed before they resume their sport. Even once they return to the sport, they use these tools constantly to heal any recurring tendon, ligament or muscle damage before it can build into something big. For professional athletes, having the right tools means all the difference. For the rest of us having the right tools should be just as important.

The human body will use scar tissue as a temporary solution and will try to build the scar tissue as fast as possible to heal tears in soft tissue (muscles, fascia, tendons). Scar tissue can form fast to bring together the edges of a tear, but working fast doesn't mean that the job's done right. When scar tissue forms it doesn't come together as neatly as regular (healthy) tendon/ligament tissue would. Scar tissue fibers lay down in a jumbled mess - just like a mass of tangled hair.

This is how scar tissue works - it is messy and inelastic. The scar tissue that forms in your soft tissue will be unorganized and won't line up properly with the healthy tissue surrounding the tear. This scar tissue will attach to everything in and around the tear including the surrounding healthy tissue as well. This results in a long-term fusing together of everything the scar tissue touches, reducing your ability to move and making the injury even more painful!

The longer a soft tissue injury remains, the more scar tissue is likely to build up in and around the injury location. This is why it is critical to treat your injury quickly without delay.

Continuous re-injury and build-up of scar tissue while staying active or going back to work means you will experience a greater risk of winding up with on-going, chronic foot pain and overcompensation injuries to the hips, knee(s), and or back.

Plantar Fasciitis Injuries Certainly Are Life Changing, Are They Not?

If you've been living with the agony of a significant foot strain for some time now, it's almost a sure thing that you're feeling discouraged, tired, and worn-out. Seemingly simple chores are now impossible - like getting in and out of seats (or your car), using a hammer/scredriver, walking the dog or just standing and walking for significant periods of time. The constant pain has no doubt interrupted your sleep, just adding to the long-term misery that this condition can bring on.

It is challenging to keep from re-injuring connective tissue in the foot, once it has been injured significantly.

It's just not possible to stop life's activities for weeks/months to rest. What ends up happening is we continually re-injure the foot through our daily activities - further setting back our recovery. The foot deals with massive amounts of tensile stress - once there is tissue breakdown, it can easily persist and worsen, especially with inadequate recovery protocols wherein reinjury and subsequent, ongoing scar tissue proliferation becomes more likely.

Plantar Fasciitis Can & Will Likely Lead to Other Conditions and Injuries.

Every time we try and use our injured foot, the damaged ligament moves; when this happens, you will likely experience severe pain. With injured tissue, pain happens - sometimes a LOT of pain, so we try not to move it. So, when doing something we need to use our plantar for (climbing stairs, getting up from the sofa, standing at the kitchen counter), we start to use other muscles more (instead of the muscles that are causing pain). Our body will automatically try to use other muscles to reduce pain even though it is inconvenient and you may not be fully aware it is happening. This concept of using other muscles/joints to complete a task is known as overcompensation.

A couple days later - after purposefully avoiding movement of our damaged foot, the pain and swelling have gone down and this is where the trouble begins! The pain disappeared with the swelling but we still aren't fully healed! Not knowing this, we start using our foot normally again and then the injury flares up again like it did before - all because the injury wasn't fully healed in the first place. So the pain comes back and now we start overcompensating yet again.

Eventually, we start overcompensating more and more in an attempt to lessen the pain. The problem is now that many of the muscles and joints you are relying on due to overcompensation are now being overused. This means that aches and pain now become commonplace in areas that you weren't expecting - all as a result of the original muscle injury and the body's instinctive nature to "protect" the orginal injury A typical example with plantar fasciitis in the right foot would be both knees, the hip and lower back are radiating pain due to the avoidance of putting your weight on the right foot metacarpals and toes (metacarpal is the ball area of the foot that takes the weight when you lift your heel off the ground).

Everything in the human body is connected. Any soft tissue injury can lead to other injuries over time if not treated properly. You might start using your opposite arm or leg to perform normal daily activities, like carrying grocery bags or moving things around your house. You may also try to use the opposite side of your body to get back to work sooner to avoid waiting for your injury to heal completely.

Many of our clients have experienced pain in other areas of their body because they shift the "work" to the opposite side of their body. For example, a serious muscle strain in the right forearm can often lead to over-straining of the left forearm (a roofer would switch from holding a nailgun in the dominant, injured arm to the other weaker arm and likely strain that forearm quite easily). Most commonly, the injury occurs on the dominant side, so the risk of straining the other (weaker) side increases. The longer the injury (and corresponding pain) persists, the greater the chance that you will sustain more strain from overcompensation, eventually leading to issues in those areas as well. To minimize potential secondary injuries, know that it is important to deal with your soft tissue injury quickly and completely.

Body imbalances result, placing tension on bone and softer tissues - leading to a misalignment within the body. The extra stress on the opposite side of the body will also result in weakness and fatigue of soft tissue - increasing the chances they can eventually be injured or gradually degenerate. This is a textbook example of how you get an overcompensation injury. In some cases, physical therapists warn that overcompensation can be very dangerous - in the lower body - especially in the foot and knee - it can affect gait or alignment on a permanent basis.

Even if you try to avoid over compensation pain in your healthy joints you're still at risk for re-injuring your soft tissue tear. Ignoring overcompensation pain and the pain in your original injury while returning to regular activities or your job can lead to even more problems with healing - like a chronic ligament injury or the formation of massive amounts of scar tissue in and around ligaments & tendons. Soft tissue tears can turn into a vicious cycle - which is why it is absolutely critical to focus on methods that improve and strengthen injured tissue in a timely manner. The faster you truly heal, the less chance you have of sinking into a downward spiral of re-injury, scar tissue growth and overcompensation injuries.


Okay, So I Have Plantar Fasciitis...What Should I Do Now?

Stage 1: If the injury is recent and you are experiencing some inflammation (unlikely though with plantar fasciitis) - use a good quality cold pack for inflammation reduction and Arnica Pain Relief Cream for pain reduction. If the injury has been going on for awhile, then skip to Stage 2 below.

If you are experiencing pain and swelling in the very early stages of the injury (first 48 to 72 hours), the first step in a conservative treatment protocol would be to focus on reducing the discomfort by applying a Cold Compress or Ice Pack to the injured muscle along with a high quality pain relief cream such as our Arnica Infusion Cream.

Not only will you use your Cold Compress or Ice Pack for the first 48 to 72 hours after the injury, but you should also use it during other stages of the healing process. Some examples include...

  • after any activity that stresses your injury in a moderate way. It is wise to quickly apply cold to not only reduce the pain, but to also minimize tissue damage that may have occurred during the prior event.
  • If you have a job that requires you to place some demands on your injury. If that is the case, use cold immediately following work and if the opportunity allows - use it for 15 to 20 minutes during lunch or another break time.
  • If you are in a post-operative situation where cold compression would be utilized heavily for at least the first few weeks following the procedure. Similarly, if you are scheduled for upcoming surgery, then you will need some type of cold temperature treatment to use so surgery can be initiated with minimal inflammation.

Having a Cold Compress or Ice Pack available at home to use if and when the need arises is helpful. Many of you already do so. However, too many people over-focus on cold while ignoring the most critical aspect to healing - nourishing the injured tissues with a healthy supply of blood flow needed to repair and rebuild the injured tissues.

When Should I Use My TShellz Wrap During the Day?
